In 2019, HB 120 and State Board Rule R277-623 was passed, requiring school districts in Utah to provide surveys to parents, students, faculty, and staff to provide feedback on suggested areas of improvement or to highlight school successes. Districts are required to distribute the surveys, which will consist of the following number of questions:
Student Survey:
Grades K-2 Grades 3-5 Grades 6-12
12 Questions 20 Questions 44 Questions
Adult Survey:
Administration School Staff SRO Parent
54 Questions 52 Questions 20 Questions 33 Questions
Parents should have received an “opt-out” form from their building administrator if they do not want their students to participate in the survey.
